Designated person* under the Orders Requiring Vaccination or Testing for Designated Persons
Staff who have direct contact with residents/clients in congregate living settings/residential care facilities
Persons experiencing homelessness or under-housed
Pregnant individuals
Unvaccinated or partially vaccinated AND 40 years of age and older

*Designated persons include: health care personnel who have direct contact with patients/residents/clients; school personnel or licensed child care facility personnel who have direct and ongoing or prolonged contact with pupils/children; civil servants in direct contact with vulnerable populations; child and family service personnel who have direct and ongoing or prolonged contact with children; funded agency personnel in direct contact with vulnerable populations; home care workers; paramedics and emergency medical responders.

Do you have symptoms of Covid-19?

Common symptoms may include: Fever/chills, cough, sore throat/hoarse voice, difficulty breathing, loss of taste or smell, vomiting/diarrhea for more than 24 hours, nausea or loss of appetite, poor feeding (if an infant), runny nose, muscle aches, fatigue, headache, pink eye (conjunctivitis), skin rash of unknown cause.

Do you have an immune system weakened by disease or medical treatment, such as any of the following?

Active treatment for solid tumor and hematologic malignancies
Receipt of solid-organ transplant and taking immunosuppressive therapy
Receipt of chimeric antigen receptor (CAR)-T-cell or hematopoietic stem cell transplant (within 2 years of transplantation or taking immunosuppression therapy)
Moderate or severe primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich syndrome)
Advanced or untreated HIV infection
Active treatment with high-dose corticosteroids (i.e., ≥20 mg prednisone or equivalent per day when administered for ≥2 weeks), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive
