Your Social Insurance Number is essential. Please ensure it is correct.
The GED Testing Office should be notified immediately of any change of address.
Give name in full. If you wrote your GED tests under a different surname, please include this surname in the “additional information section” of this form. You can also indicate in this section, fax requests, or additional mailings.
A fee of $31.50 must be submitted with the application form.

- $31.50 - Total Payment Due
This fee is NOT refundable. Please note, credit cards will not be charged, nor will payment be cashed until such time the search indicates there is a record on file.
